Did you recently leave DCS custody? Are you under age 21? Do you need health insurance?

You may be able to get TennCare if:

• You make no money or very little money, or
• You have unpaid doctor bills, or
• You are pregnant, or
• You have a child or take care of a younger sister, brother, cousin, etc.

*If you are under age 19 and you can’t get on TennCare Medicaid, you may be able to get on TennCare Standard*

How to apply for TennCare

You need to fill out the application papers.

• Apply at your local Department of Human Services (DHS) Office. You can find your local DHS office here: http://tn.gov/humanserv/st_map.html OR:
• You can also get the papers at your local DHS office and send them in by fax.
